COVID-19 TESTS & RESULTS:

If you have Covid-19 symptoms or are a household contact of a positive case please go to https://nrhcc.health.nz for up to date  information. Most testing is now done via Rapid Antigen Testing (RAT TEST) with results available within 30mins. You will need to self-report your result via My COVID Record or by calling 0800 222 478 and pressing option 3. 

The majority of positive cases will be able to self-manage and be required to follow current isolation rules. If you have concerns or if your symptoms are worsening, call us on 09 486 2122 option 2 or 111 in an emergency.

 

 

 

Patient and Staff Safety:

Please call 09486 2122 ext 2 if you are unwell and need an appointment. If you have developed Covid type symptoms, please complete a Rapid Antigen Test (RAT Test) at home before coming into the surgery.  PLEASE DO NOT COME TO THE CLINIC UNANNOUNCED.

 PRESCRIPTIONS:  You may order a repeat prescription online through our Patient Portal, Manage My Health, or leave a medication request on the nurses answerphone on 09486.2122 ext 1 with your:  Name, Date of Birth, cellphone number and Pharmacy Name.

Your account will be sent either by text or email for you to pay online. Please allow 24 hours for your script to be sent to your preferred pharmacy.  


Opening Hours

Office/ Reception                        Mon - Fri 8.00am to 5.00pm.  We are now seeing patients face to face although masks are still required to be worn if you have any cold /flu type symptoms. Please advise reception if this is the case. Telephone consultations are also available on request.

We are a Family Practice located at 2, Byron Avenue in Takapuna. We have been providing high quality medical care to the community since 1958. Byron Chambers was the first purpose built Medical centre on the North Shore. Over the years we have been able to deliver health care services with an emphasis on health education and preventative Medicine to the people on the Shore and nearby. Your better health is our priority.

NEW PATIENTS

Unfortunately our books are closed at present.

When we are open for new enrolments, you will need to register with our practice by filling out an Enrolment Form prior to booking your first appointment. 

PLEASE NOTE: We require a minimum of 48 hours to process your enrolment. No appointments can be made prior to this.

If you are not an NZ citizen you may not be eligible for government subsidised care. However, non-eligible patients are able to register and access full medical care for an unsubsidised fee. See our Fees page for more details on consultation rates.

To be eligible for enrolment subsidised care you must meet one of the criteria listed on the back of the enrolment page. You can also find the criteria at the Ministry of Health website – if you are not an NZ citizen, we recommend that you read this as you may still be eligible.

If you are an NZ citizen (including Cook Islands, Niue or Tokelau) to enrol at our practice and receive subsidised consultations, you must provide us with a valid form of ID for enrolment such as:

·         your New Zealand passport OR

·         your New Zealand Birth Certificate (or Cook Island, Niue or Tokelau birth certificate) AND two forms of proof that you are the person on the birth certificate

Examples of other identity documents include:

·         a driver licence

·         an 18+ card

·         a Community Services Card or Super Gold Card

·         a School/Tertiary ID Card

If you are not an NZ citizen please bring your passport and your visa(s).
